Although insurance companies offer coverage for bariatric surgery for patients with BMI > 40 or BMI 35-39.9 with one or two co-morbidities, some employers may exclude the procedure from the policies they offer to their employees.

If your employer has not excluded bariatric surgery, your insurance company may require a surgical preparatory program that may include:

  • Psychological evaluation
  • Nutrition consultation
  • Physician supervised weight loss program verification

Typical time frame for insurance approval is 3 weeks to 7 months, depending on your insurance company and employer specific plan.
